• Hello everyone! My name is James Hall and I am a Family Nurse Practitioner. I was one of the providers who routinely discouraged my patients from using a Ketogenic diet over the years, thinking it was unhealthy and just a fad diet like so many others. I recommended low fat diet, exercise more, and advocated calories in vs calories out and when it didn't work, I blamed the patient for not trying hard enough. Diabetes runs in my family and over the years, as I put on weight, I found out I had type 2 diabetes as well. I had used diet pills, low fat diet, and restricted calories but it never lasted and always came back, usually weighing more than when I started. I exercised regularly but found it more and more difficult to lose weight or control my diabetes, even on medication. I saw a documentary on how ketogenic diet could successfully control diabetes and help with weight and I speechless. I began reading research and learning as much as I could and never realized how many metabolic illnesses were interlinked! I started doing a ketogenic diet and at 3 months I had lost 44 lbs, my A1C had dropped to 5.3, my vitamin D deficiency was gone, my TSH was normal off medication, my lipids were normal off medication, and I no longer needed my C-pap. I started talking to my family and friends and showing them my results and they also tried ketogenic lifestyle and have all had similar results! I now recommend it to my patients and have seen dramatic improvements such as deprescribing statins, insulin, and other oral diabetic medications as well as significant weight loss, some as much as 50 lbs in just three months. I'm very excited to share this with friends, family, and patients and I feel like I am actually making a difference again.

• My name in Mike Jones. I am a board-certified obesity medicine specialist and am in the process of clinical lipidology and physician nutrition specialist board-certifications. I practice full-time obesity/metabolic medicine in Lynchburg VA. My goal is to create a specialty practice that reaches beyond simply treating obesity (as lofty an endeavor as that can be) in isolation and expand into the active treatment/management of many of its comorbidities (NAFLD, Lipid disorders, ASCVD risk reduction, T2DM, etc.). I would love to hear thoughts, particularly from anyone already doing this type of expanded practice.

• In one of the Nutrition Network lectures by Megan Ramos, she differentiates Time-Restricted Eating from Intermittent fasting, where TRE includes eating 3 meals but condensing each eating window to only 60-90 minutes thus no snacking in-between meals but also no absolute feeding window. IF is more along the long the lines of an overall condensed feeding window with roughly 2 meals and optional snacking in-between. That was the first I've heard it stated that way, thought it was interesting. TRE is a useful concept for patients who are hesitant or afraid to go too long without a meal. So with this approach they focus on learning how to cook healthy, satiating meals on their journey to trying IF.

• In the context of SMHP leaving behind "low-carb" in it's title and encompassing a broader approach to metabolic health, it think the term "Therapeutic Nutrition for Metabolic Homeostasis" is useful for those who need medical intervention, or"Therapeutic Nutrition for Metabolic Optimization" for those who are healthy and looking to improve performance.

• Woot! I love that one too, but I've also learned that "Therapeutic Carbohydrate REDUCTION" causes less of a knee-jerk reaction from dietitians who don't think any foods should be "restricted."

• Hello to all, I am thrilled to meet all the folks who have introduced themselves. I'm a registered dietitian with a background in public health and nutritional epidemiology (and a PhD in rhetoric & communication). I had the great honor of working with Dr. Eric Westman when he opened his doors at the Duke Lifestyle Medicine clinic back in 2006. I've been in and around the low-carb universe for a while now, and I'm currently part of the DietDoctor.com team. One of my unofficial jobs there is to find out what clinicians (and their patients or clients), researchers, and other low-carb practitioners need from us and try to provide it. • Hello Everyone, I am very excited to be part of this new organization. I am a Allergist/Sinus Surgeon by trade but have been studying mitochondrial medicine for the past 7 years.
