• Great article, but I would add that vitamin D deficiency is more likely in people with dark skin, which might provide another explanation for the disparities in outcome. We know that vitamin D is important for proper immune functioning.

• Hello Peter, Thank you very much for sharing your thoughts about the topic of insulin resistance and glucagon. I have try combining resistance training with low intensity exercise (walking 3.5 miles in about 70 minutes daily). Honestly, i am not a big fan of resistance exercise but when trying this option, i had muscle damage enzyme increased appreciably. Maybe I was mot doing the proper resistance training or simply that there was a clinical lab error. At the end, i was discouraged to continue resistance and I am just walking now. Do you have any suggestions for me, Peter? I have not been able to access your case study report. Can you resend the link. Thank you so much!

• Luis do you use resistance training on a regular basis? I believe you are right about glucagon's role in what is happening. Part of our work load for the NN Practitioner course which I just finished, was a case study. I chose to write mine on just such a case where after fabulous initial results, including discontinuing all glucose and bp medications in five weeks, 30 pound weight loss in 90 days, normalization of HbA1c and fasting glucose readings into the 100 mg/dL range, this patient spontaneously lost an additional 8 pounds, an additional inch off his waist and saw BP levels drop further into the low normal range and began reporting fasting glucoses in the 80's for the first time. When I say spontaneously I mean no further changes in diet or activity. This was after one year on our program. I asked you about resistance training as this patient seemed to be very sensitive to resistance training. Prior to beginning our comprehensive lifestyle program he was consistently walking 20,000 steps daily. At 70 years of age he began our program in the excellent cardiorespiratory fitness classification. This is uncommon in this population. We saw major improvements with the implementation of LCHF but what was interesting was time off from resistance work during COVID PHE and then the restart, then time off for a knee scoping, and then a restart. Each time resistance training was started and stopped and started again we saw a notable change in his fasting glucose readings. As for hypothesis as to the reason for the spontaneous changes after one year, the instructors of the NN and I feel that beta cell function was improving therefore shutting down glucagon production. The reason I used this patients case study for my paper was to point out these issues and suggest more work is needed. Be patient. All cells heal and regenerate at different rates in different people.
